Registration Requirements
Any newborn of a currently registered person who is permanently resident within our practice boundary may apply for registration with the practice. It is advised that you register your baby with your GP as early as possible in case you need their help.

During covid restrictions registration forms should be emailed to the practice administration email account - gram.oldmachar.registration@nhs.scot
PLEASE NOTE: YOU NEED TO COMPLETE BOTH THE APPLICATION TO REGISTER DOCUMENT AND THE ETHNICITY REGISTRATION DOCUMENT WITHIN THE PACK TO REGISTER WITH THE PRACTICE.

Preference of Practitioner
Since April 2004, all patients in the UK are now legally registered with the practice and not an individual GP. For administrative and continuity reasons however the Old Machar Medical Practice has decided to retain the practice of assigning each patient to a "Usual Doctor" who is ultimately responsible for their medical care. You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. We will do our best to respect your choice however, not all the doctors in the practice provide all services and specific doctors may not be immediately available. The practice retains the right to insist that certain conditions/patients should be seen by specific practice personnel.
