Jesmond Surgery is currently full
At present our list for Jesmond Surgery list is operating on an "open but full" basis. We are operating a waiting list for those that wish to register and will be registering patients on a "one on for one off" basis. Other local practices continue to have capacity for registration at this time.
Registration at the King Street branch remains unaffected.
Any person permanently resident within our practice boundary may apply for registration with the practice. When registering, it is essential that we see two forms of identification. One form of identification should be photographic where possible or a birth certificate, and the other form of identification must confirm your current address e.g. utility bill, rental agreement, or bank statement. This is a requirement for ALL persons registering with the practice. You will be asked to complete a registration form for each person registering with the Practice. In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to Old Machar Medical Practice.

During covid restrictions registration forms should be emailed (along with your 2 forms of identification as detailed above) to the practice administration email account - firstname.lastname@example.org.
PLEASE NOTE: YOU NEED TO COMPLETE BOTH THE APPLICATION TO REGISTER DOCUMENT AND THE OLD MACHAR PATIENT REGISTRATION DOCUMENT WITHIN THE PACK TO REGISTER WITH THE PRACTICE.

Preference of Practitioner
Since April 2004, all patients in the UK are now legally registered with the practice and not an individual GP. For administrative and continuity reasons however the Old Machar Medical Practice has decided to retain the practice of assigning each patient to a "Usual Doctor" who is ultimately responsible for their medical care. You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. We will do our best to respect your choice however, not all the doctors in the practice provide all services and specific doctors may not be immediately available. The practice retains the right to insist that certain conditions/patients should be seen by specific practice personnel.
