Any person permanently resident within our practice boundary may apply for registration with the practice. When registering, it is essential that you bring two forms of identification and if possible your NHS medical card. One form of identification should be photographic where possible or a birth certificate, and also another form of identification such as a utility bill, bank statement or any form of documentation that confirms your current address. This is a requirement for all other persons registering with the practice. You will be asked to complete a registration form for each person registering with the Practice. In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one.
Only patients over the age of 45 years and those on repeat medication need to be seen by the Practice Nurse and/or a doctor to complete registration with the practice. Our reception staff will be happy to assist you in organising the necessary appointments.

Preference of Practitioner
Since April 2004, all patients in the UK are now legally registered with the practice and not an individual GP. For administrative and continuity reasons however the Old Machar Medical Practice has decided to retain the practice of assigning each patient to a "Usual Doctor" who is ultimately responsible for their medical care. You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. We will do our best to respect your choice however, not all the doctors in the practice provide all services and specific doctors may not be immediately available. The practice retains the right to insist that certain conditions/patients should be seen by specific practice personnel.
